Home Health
The Bremer County Health Department provides non-certified home health services. 

Our Home Health programs include:

Skilled Nursing:  A RN or LPN provides services under your physician's orders such as: wound care, catheter or colostomy, blood draws, assistance with medication management, post-hospitalization follow-up, medication injections, influenza immunization, plus more.

Personal Cares: A CNA provides services such as bathing, shaving, dressing, range of motion exercises, and meal preparation.

Homemaker:  A CNA provides light housekeeping, meal planning and preparation, shopping for food and essentials, and laundry.  For details regarding this program and whether you qualify, contact us at (319) 352-0082.

Funding and Payment Options:  For those who qualify, the BCHD provides home health services for individuals who are able to pay privately for services based off our current costs.  Additionally, a grant from the Iowa Department of Public Health allows us to offer services on a sliding fee scale so that all Bremer County residents who need home health have access to it without being financially burdened.

Referrals: Referrals are accepted from anyone and anyone who is eligible if the need exists.  There is no cost for the confidential needs assessment.  Services are determined by the participant with a BCHD representative.  Call (319) 352-0082 to make a referral between the hours of 8:00 AM- 4:30 PM Monday-Friday.

BCHD Home Health services are available  only to Bremer County residents.
